Hashag Health needed a check-in system that frontline care workers would actually open every day. We built a mobile-first wellness app with streak mechanics, clinical-grade data capture, and a care team dashboard — and got adoption above 80% in week one.

Hashag Health was running wellness programs via paper forms and weekly email surveys. Response rates hovered at 12%. Leaders couldn't identify at-risk individuals early enough to intervene.
The app had to be fast (under 60 seconds to complete), feel personal, and produce structured data that clinical managers could act on. It also needed to comply with data handling standards for health information across multiple jurisdictions.
Previous attempts at digital check-ins had failed because they felt like admin burden. The product had to feel like a habit, not a task.
60 seconds or it fails.
We ran co-design sessions with 24 frontline workers across three hospitals. The insight: any check-in that takes more than 60 seconds gets abandoned. We designed a 3-question flow with streak rewards, optional journaling, and a supervisor-visible burnout score calculated server-side — never shown to the employee.
A habit loop built on clinical data.
The mobile app is Flutter — single codebase, native feel. The clinical dashboard is a React web app with role-based views for team leaders and org-level HR. Check-in data is encrypted in transit and at rest, with consent-gated sharing that workers control.

Visual streak counters, milestone badges, and team leaderboards keep completion rates above 80% after month one.

Optional daily prompt with privacy-mode journaling — workers can process stress without it reaching supervisors.
Burnout risk scoring
Proprietary risk model flags individuals approaching burnout thresholds. Only supervisors see risk level, never raw responses.
Clinical manager dashboard
Real-time team wellness heatmap, risk flags, trend charts by ward/department, and one-click intervention notes.
Push notification pacing
Smart delivery timing based on each worker's shift pattern — check-ins arrive post-handover, not mid-procedure.
Consent management
Granular opt-in for each data type. Workers can withdraw sharing at any time. Every consent change is logged.
HR org-level reporting
Aggregate wellness trends by site, role, and shift — no individual data — for board-level wellbeing reporting.
Offline support
Check-ins queue locally when off-network (common in basements and theatres) and sync when connectivity returns.

We chose the boring, battle-tested options where it mattered and innovated where it gave the product a real edge.
We launched on a Monday. By Friday, 82% of our floor had completed at least one check-in. I've never seen adoption like it.
What it took.
- Workers don't trust wellness data. We ran trust-design workshops before any code. The result: raw responses never reach supervisors — only a risk tier (green/amber/red). Workers control what they share.
- Shift patterns break standard notification logic. Nurses on nights, rotating shifts, and 12-hour blocks need notifications at different times. We built a shift-aware delivery scheduler fed from the HR system.
- Offline in clinical environments. Signal is unreliable in theatres, basements, and isolation wards. The app queues locally and syncs silently — workers never see a failed submission.
- Data residency across jurisdictions. The platform operates across multiple countries with different health data laws. We built per-region data residency routing with a compliance dashboard for the legal team.
